Your Opportunity
Youth on Track is an early intervention program supporting 10-17-year-olds who are at risk of long-term involvement in the justice system. The program works with young people and their families through case management and evidence-informed interventions aimed at reducing criminogenic risks and addressing individual needs.

This is an on-site position based across both Kempsey and Port Macquarie, with outreach work forming a key part of the role. As a Senior Case Manager, you will provide intensive case management supports, deliver a range of evidence-based interventions, and coordinate services to help young people build resilience, achieve their goals, and divert from the criminal justice system.

Due to the geographical area the program covers, outreach work is an essential part of the role, and a pool car is provided.

Your Key Responsibilities

  • Respond to referrals and conduct timely intake and risk assessments in line with program guidelines.
  • Deliver intensive case management to young people and families addressing criminogenic risks and needs.
  • Develop, implement, and review individualised and family case plans that focus on behavioural, social, and cultural needs.
  • Provide evidence-based interventions addressing factors such as substance use, family functioning, education, and housing.
  • Build and maintain trusted relationships with young people, families, and key stakeholders including police, schools, and courts.
  • Ensure accurate documentation, reporting, and data entry to support program outcomes and compliance.
  • Provide culturally responsive and sensitive support to clients from diverse backgrounds.

Requirements for Success

  • Relevant tertiary qualification (Cert IV or higher) in Youth Work, Case Management, Social Work, Psychology, Criminology, or equivalent experience.
  • Minimum of 2 years' experience in intensive case management, preferably with young people in the youth justice or mental health sector.
  • Strong understanding of best practice criminogenic interventions, behavioural supports, and trauma-informed approaches.
  • Demonstrated ability to manage a caseload, prioritise tasks, and work effectively in dynamic environments.
  • Ability to travel between Kempsey and Port Macquarie and provide outreach support as required.
  • Strong communication, engagement, and relationship-building skills with young people and their families.
  • Ability to work both autonomously and collaboratively as part of a multidisciplinary team.
  • First Aid Certification is desired.
  • Must have a Full driver licence with no restrictions.
