About the role

Permanent full time opportunity to work autonomously, understands the health system and organisation and is able to manage programs. This position requires an experienced and senior practitioner with the skills to provide independent after hours on call emergency care; professional capabilities to deal with complex situations; and the ability to manage primary health care programs within a holistic model focused on improving outcomes for remote clients.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Provide leadership to the health care team and community on the provision of a holistic health care service in a local primary health care community environment to build both cultural and acute
  • Manage and influence culturally responsive primary healthcare services/programs in a community or Hospital and Health
  • Participate as a senior member of a multi-disciplinary management team in the development, implementation and review of health care and case management plans to ensure provision of a consistent and agreed approach to health
