About the role

At MATSICHS Morayfield Clinic, we are committed to delivering culturally responsive, holistic primary care to Aboriginal and Torres Strait Islander communities. Using the innovative IUIH model of care, we focus on providing continuity of care and improving health outcomes for our clients.

We are looking for a passionate and flexible Health and Wellbeing Worker to support our mission. In this role, you will help guide clients toward autonomous access to essential health services, providing both information and hands-on support to meet their unique needs.

Key Responsibilities:

Offer expert advice, resources, and guidance to clients and their families, connecting them to the right services within the IUIH network.

  • Educate clients and families on the roles of various healthcare providers and how they work together to support holistic care.
  • Serve as the first point of contact for clients in times of heightened need, helping to prevent or address crises with compassion and care.
  • Collaborate with clients and families to set health and social goals, ensuring their needs are prioritised.
  • Work alongside a dedicated multidisciplinary team to implement personalised care plans, ensuring the best possible outcomes for each client.
