About the Role

We are seeking a Client Services Coordinator (CSC) to join our team in Glenelg East on a 9-month maternity leave contract, with the potential for full-time employment beyond the contract. In this pivotal role, you will be responsible for coordinating high-quality, person-centred services for people with disability, ensuring their goals and wishes are always at the forefront.

You will:

Ensure participants receive the agreed level of support as outlined in their service agreements

Coordinate and manage services using a person-centred approach

Apply case management skills to oversee participant supports and outcomes

Liaise with participants, families, support workers, and external providers to deliver seamless support

Escalate and resolve service delivery issues appropriately

Prepare reports, track outcomes, and maintain compliance with legislation and policy

Manage low-level complaints and incidents with professionalism and care

What We’re Looking For

We are seeking someone who shares our values and brings:

Case management experience in the disability or community services sector (essential)

Strong knowledge of the NDIS and disability services sector

Relevant qualifications in disability, community services, healthcare, or aged care (or experiential equivalent)

Excellent communication and interpersonal skills, with the ability to build strong relationships

Ability to problem-solve, prioritise, and manage competing demands effectively

Strong decision-making and report-writing abilities

Proficiency with software and technology used in service coordination

Mandatory Requirements

NDIS Worker Screening Check

Current Driver’s Licence (min P2)

First Aid Certificate and CPR

Full working rights in Australia
