About Us: Allied Connect is a multidisciplinary allied health practice dedicated to providing community-based rehabilitation services across diverse client groups, including aged care. With a varied caseload covering neurological, orthopedic, cardiorespiratory, and post-surgical or deconditioned clients, our team members experience rewarding, hands-on work that impacts lives daily.

The Role: This role offers a mix of community and residential care, with responsibilities such as:

Visiting community clients to assess, report on needs, and create goal-focused therapy plans.

Scheduling client visits efficiently, particularly for those familiar with QCSS, STRC, TCP, CHSP, NDIS, HCP, SAH & DVA.

Working closely with residential aged care facility staff to assess residents upon entry, create therapy plans and oversee wellness and reablement programs.

You’ll also monitor and adjust equipment needs, work on falls prevention strategies alongside a multidisciplinary team, and lead one-on-one or group therapy sessions based on client needs and your expertise.
