About the Role
As Care Manager, you will be responsible for managing client relationships and overseeing the delivery of services to ensure compliance with the Aged Care Quality Standards. You will work closely with clients, families, and care staff to deliver an exceptional home care experience and support clients to live independently at home.

Key Responsibilities

• Complete client assessments and develop personalised care plans
• Oversee service delivery and supervise staff to ensure quality standards are met
• Review client progress notes, implement corrective actions, and manage incidents
• Build and maintain strong relationships with clients and their families, seeking regular feedback
• Meet with prospective clients and explain the provider’s value proposition
• Collaborate with scheduling and operations teams to ensure client and staff needs are met
• Chair team meetings and conduct staff performance reviews

About You

• AHPRA-registered professional (Registered Nurse or Enrolled nurse)
• Compassionate, reliable, and highly organised with strong interpersonal skills
• Experienced in care coordination, compliance, and supporting aged care clients
• Ability to collaborate effectively within a multidisciplinary team
• Passionate about improving outcomes and providing the highest standard of care
