The role:
Join Avivo's small, supportive nurse team and make a real difference in your community. This role offers variety and autonomy, supporting customers across the Perth Metro area with home visits, health care planning, and clinical training for support workers.
We're especially looking for someone based in the northern suburbs to balance our team.
What you'll do:
- Provide wound, tracheostomy, PEG, catheter (supra pubic & indwelling) and diabetes care
- Review and update health care plans
- Mentor support workers and external agencies in clinical skills
- Liaise with families, coordinators, and other health professionals
- Travel across Perth Metro (north–south)
What you need:
- Current AHPRA RN registration
- 5+ years' clinical experience
- Strong documentation and computer literacy
- Excellent communication and training skills
- First Aid/CPR & NDIS Worker Screening (or willingness to obtain)
- Valid WA Drivers Licence
- Community nursing experience is desirable, not essential
