The Role
As our Client Service and Intake Coordinator, you’ll be the welcoming face of Rocket Care—managing referrals, coordinating services, and ensuring a smooth and supportive transition for new participants. You’ll play a vital role in connecting clients and their families with the right services, while keeping everything compliant, accurate, and person-centred.
Key Responsibilities
Triage and manage incoming referrals and service pipelines
Coordinate client onboarding, service agreements, and intake documentation
Build and maintain strong communication with clients, families, and stakeholders
Develop care profiles, support plans, and ensure smooth service commencement
Monitor NDIS plan dates to ensure continuity of supports
Manage client incident reports and compliance requirement
Collaborate with the Workforce Coordinator to match the right staff with the right clients
Provide after-hours support as part of the Manager On-Call rotation
What You’ll Bring
Strong organisational and case management skills
Excellent communication and interpersonal abilities
Knowledge of NDIS processes and compliance standards
Ability to manage competing priorities and work under pressure Proficiency with client management systems (Sandwai experience desirable)
Strong problem-solving and decision-making skills
