Wholesome Solutions Pty Ltd is seeking dedicated NDIS Support Coordinators to join our team. We currently have two positions available - one part-time and one casual to begin with. Offering the opportunity to make a meaningful impact in the lives of individuals living with disabilities, you will play a vital part in navigating the NDIS system and ensuring our clients receive the tailored support they need to thrive.

What you'll be doing

Coordinate and facilitate the implementation of NDIS plans for clients, ensuring their involvement every step of the way to meet their goals and needs

Liaise with service providers, government agencies and other stakeholders to coordinate and monitor the delivery of supports

Assist clients in understanding their NDIS plans and the available support services

Advocate on behalf of clients to secure appropriate and effective support services

Regularly review and update client NDIS plans and budgets to reflect changing needs and circumstances

Maintain detailed records and documentation to support client progress and service delivery

Collaborate with the broader Wholesome Solutions team to provide holistic care and support

The option to perform some company administration duties to increase hours while you build up your case load.

What we're looking for

Experience preferred but not essential — ideally in Support Coordination, Case Management, or a similar role. Experience within the NDIS space is highly regarded, and relevant transferable skills will also be considered.

Demonstrated understanding of the NDIS and experience navigating the scheme on behalf of clients

Relevant diploma / tertiary qualification in a human services or allied health discipline is advantageous

Bring your current caseload - we welcome support coordinators who would like to bring along participants, whilst having the capacity to take on more as referrals come through.

Excellent communication and interpersonal skills, with the ability to effectively liaise with a range of stakeholders

Strong problem-solving and critical thinking skills to identify and address client needs

Commitment to providing person-centred, empowering support to clients with diverse needs

Willingness to travel within the local area to meet with clients as required

Current driver's licence and access to reliable transportation

Current NDISWC and Working with Children Check
