About the role

An exciting opportunity for a Support Coordinator to join our amazing Heart2Heart Team in Toowoomba. This position is a Part Time position working 9am-3pm Monday - Friday.

The Support Coordinator will apply attentive interpersonal skills, an understanding of the NDIS, time management, and planning and administration, to enable each NDIS participant to access the supports and services they need to lead everyday lives, and to reach their personal goals.

Conduct initial assessments of participants, interpreting NDIS plans and health information

Provide appropriate intake, support, information and referral of eligible participants

Promote empowerment, choice and control and community inclusion of people with disabilities and their families, consistent with their NDIS plan

Ability to attract and retain the targeted population of NDIS customers

Be a key point of contact for the participant for the NDIS and all services and supports to ensure the full implementation of the participants NDIS plan

Provide Level 2 Coordination of Support with a minimum of 80% caseload hours - supported by an existing and buildable caseload of Participants

About You

The successful applicant will have excellent time management skills, flexibility and willingness to grow with the organisation as well as;

Minimum 1 year experience in Support Coordination (preferred) but willing to support someone who has worked in the Disability Industry and has transferable skills

Detailed working knowledge of relevant community networks and relevant external services

A strong passion for helping others to learn and grow, a positive attitude, show initiative, are dedicated, and have a professional and personalised presence

Adaptability and ability to contribute in a positive and proactive manner within a multidisciplinary team

Empathy and understanding for clients and staff while maintaining emotional resilience
