West End Support Services, established in 2017, is a locally owned and operated registered NDIS provider based in the heart of Winter Valley, Victoria. Founded by Managing Director Debbie Jew, our organisation was built on a strong passion for delivering personal, participant-first services to people living with a disability.

Support Coordinator

We are currently seeking a passionate and experienced Support Coordinator to join our dedicated team. This role is pivotal in helping NDIS participants build capacity, connect with services, and navigate their plans with confidence and clarity.

Requirements

Your primary role will be to work directly with participants and support them through coordinating and managing their NDIS plan, budgeting and liaising with providers and stakeholders.

This position will require an individual who has:

Minimum of one years' experience as a Support Coordinator or Case Manager

Demonstrated ability to work confidently and autonomously.

Effectively manage deadlines and priorities for multiple clients simultaneously.

A Solid understanding of the NDIS and its Importance to People with a Disability and their Support Networks.

Ability to hold service providers accountable if support is not aligned with clients' preferences and goals.

Demonstrated ability to work with parents, carers or guardians who experience burnout, providing intensive support to ensure the best possible outcome for the client.

Knowledge of local disability, mainstream and community services.

Effective written and verbal communication and ability to prepare clear and accurate reports and correspondence that are appropriate for the purpose and audience.

Have excellent time management and previous experience achieving KPI's and monitoring billable time.

Minimum Certificate IV in Disability, or other relevant qualification
