At Better Days Support Services, we are dedicated to providing high-quality, person-centered support coordination to NDIS participants with a focus on mental health. We believe in fostering hope, building resilience, and empowering individuals to achieve their goals and live fulfilling lives.
Due to continued growth, we are seeking a motivated and compassionate Support Coordinator to join our dedicated team our office is based in Sebastopol, Victoria.
In this rewarding role, you will:
Work collaboratively with participants and their families/carers to understand their goals, needs, and aspirations.
Develop and implement individualized support plans that are tailored to each participant's unique circumstances and promote their well-being.
Assist participants in navigating the NDIS, understanding their plans, and accessing appropriate supports and services.
Build and maintain strong relationships with service providers, ensuring effective coordination and delivery of services.
Monitor the progress of support plans, review outcomes, and make adjustments as needed.
Advocate for participants to ensure their rights are upheld and their voices are heard.
Maintain accurate and timely records in accordance with NDIS guidelines and organizational policies.
Contribute to a positive and supportive team environment.
We are looking for someone who has:
A genuine passion for supporting individuals with mental health challenges and a strong understanding of the recovery-oriented approach.
Demonstrated experience in a support coordination role within the NDIS framework.
Excellent communication, interpersonal, and relationship-building skills.
Strong organizational and time management skills, with the ability to prioritize and manage a caseload effectively.
A proactive and solutions-focused approach to problem-solving.
A sound understanding of the NDIS legislation, policies, and procedures.
Proficiency in using relevant software and technology.
A relevant qualification in Disability, Mental Health, Social Work, or a related field (desirable).
A current and valid Driver's License and reliable vehicle.
A National Police Check and Working with Children Check (or willingness to obtain).
