About the role

My Village Carers' is seeking a passionate and dedicated Support Coordinator to join our team in Bendigo, Victoria. As a part-time role, this position is responsible for coordinating and monitoring the delivery of high-quality support services to older adults and individuals with disabilities within our local community.

What you'll be doing

Develop and implement personalised support plans that address the unique needs and goals of each client

Coordinate and oversee the delivery of in-home care, social support, and community engagement activities

Collaborate with a multidisciplinary team of care providers to ensure seamless service delivery

Monitor client progress and adjust support plans as needed to meet changing needs

Advocate for clients and liaise with government agencies, healthcare providers, and other stakeholders

Maintain detailed records and documentation to ensure compliance with industry standards

What we're looking for

Relevant tertiary qualifications in aged care, disability support, or a related field (preferred)

Experience in support coordination

Strong understanding of the aged care and disability support sectors, including relevant legislation and regulations

Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families

Demonstrated problem-solving skills and the ability to think creatively to meet client needs

Proficient in the use of relevant software and technology to support service delivery

A compassionate and empathetic approach, with a genuine commitment to improving the lives of older adults and individuals with disabilities
