Blue Sky Community Services are seeking to employ a professional and values-aligned person to join our supportive and friendly team and to assist us in delivering the National Disability Insurance Scheme (NDIS) Support Coordination Service in Port Stephens, Forster, Taree, Gloucester and surrounding areas (applications will be considered from all locations). This is a full-time position (38 hours per week) working from home created in response to the expansion of our service.

About the Role

The NDIS Support Coordinator:

Supports Participants receiving funding from the National Disability Insurance Scheme (NDIS) to interpret and use their NDIS plan and build capacity to make decisions and choices.

Assists participants to develop knowledge, experience and connections with the community and broader systems of support.

Works with Participants to understand Plan funding and helps identify/develop support solutions to meet the Participant's outcomes.

Coaches Participants through challenges, helps prepare for reviews and reports on achieved outcomes.

Applicants will need to have their own fully insured and registered vehicle available for work purposes with Roadside Assistance. A willingness to travel within the region is essential.
