About us
Our Veritas House Support Services team provide supports to the various programs of Veritas House - a vibrant not for profit community based organisation with a specific focus on supporting vulnerable children, young people and their families in the Bathurst and Orange areas. These are a combination of assessment, therapeutic intervention, behaviour support and consultation/advice services which help meet the needs of children, young people, families and carers of Veritas House. Additionally, they provide services to the general community, with a focus on supporting the needs of children, young people, families and participants.
The role
We are searching for an enthusiastic, flexible and highly motivated NDIS Support Coordinator to join our growing NDIS Program.
This newly created role is based in Bathurst, Permanent Part time (3 days a week) with flexibility on the hours and days.
Primary Purpose of Position
To coordinate and oversee the day-to-day delivery of supports for NDIS participants, ensuring plans are implemented effectively and aligned with individual goals. This includes managing service agreements, liaising with providers, monitoring budgets, maintaining accurate records, and responding promptly to participant needs and changes.
Key Responsibilities
Support NDIS participants to understand and implement their plans and funding.
Coordinate a range of formal, informal, and mainstream supports to meet participant goals.
Connect participants with appropriate providers and community resources.
Collaborate with participants, families, carers, and providers to monitor progress and adjust supports.
Promote participant independence and capacity to manage their supports.
Maintain accurate documentation and reports in line with NDIS requirements.
Work within a person-centred, strengths-based, and trauma-informed framework.
Monitor service agreements and ensure alignment with participant goals.
Provide crisis support and advocacy as needed.
Uphold the NDIS Code of Conduct and ensure compliance with legislation and our organisational policies.

Key Selection Criteria
Qualification in Social Work, Psychology, Allied Health, Disability, or a related field (or working towards).
Proven experience in case management, support coordination, or a related role within the disability sector.
Sound understanding of the NDIS framework, pricing arrangements, and service delivery principles.
Strong interpersonal and communication skills with the ability to build relationships with diverse stakeholders.
Excellent organisational and time management skills, including the ability to work independently and manage competing priorities.
Demonstrated ability to work within a person-centred and strengths-based approach.
Proficiency in using case management systems, Microsoft Office, and digital communication tools.
