What You’ll Do

  • Office-based role providing case management and support via phone and email to a dedicated portfolio of clients
  • Partner with clients to create, review, and adjust personalised care plans - ensuring they understand their options and make the most of their package
  • Build trusted relationships with clients, families, and providers by navigating complex issues and sensitive situations with professionalism and care
  • Confidently handle challenging conversations, guiding clients towards positive decisions that improve their independence and quality of life
  • Provide clear education around care budgets, helping clients see opportunities to access more services and get the best value from their package
  • Proactively identify risks and opportunities to enhance client outcomes, ensuring safe, high-quality care supported by accurate documentation
  • Contribute to a positive team culture through shared learning, collaboration, and a proactive approach to client success

What You’ll Bring

  • Customer service or call centre experience (case management, client support, or account management highly regarded)
  • Confidence managing fast-paced, phone-based interactions with empathy, professionalism, and influence
  • Strong communication skills, including the ability to educate and guide clients through complex conversations
  • A proactive approach to uncovering opportunities that help clients make the most of their care package
  • Calm, solutions-focused mindset when challenges arise
  • Comfortable using multiple systems, including Microsoft 365 and CRMs, with the ability to quickly learn and adapt to new technology and digital tools
  • Organised, adaptable, and able to manage competing priorities in a dynamic environment
