The Opportunity
We are seeking an experienced and compassionate Clinical Care Partner to join our Home Care Packages Program.
In this role, you will work alongside clients to deliver personalised, high-quality clinical care and support. Using a Consumer Directed Care (CDC) approach, you will develop and monitor care plans, coordinate services, and help clients achieve their wellbeing goals to remain safe and independent in their own homes.
This is a unique opportunity to play a key role in a supportive, multidisciplinary team while contributing to the future of aged care as we transition to the new Support at Home program in November 2025.
Key Responsibilities
- Meeting with potential clients and their families to navigate through the home care package process and understand their care needs.
- Partner with clients, carers, and families to develop, implement, and review tailored care plans.
- Provide clinical oversight and risk assessments, ensuring safe, evidence-based care.
- Manage client budgets within the CDC framework and coordinate services with external providers.
- Support clients through specialised programs such as Short-Term Restorative Care, End of Life Pathway, and the AT-HM program.
- Maintain accurate documentation and service delivery in line with aged care standards and regulatory requirements.
- Reporting any incidents and feedback, including SIRS as per organisational & legislative requirements.
- Collaborate with the multidisciplinary team and external providers including GPs, hospitals, and community services.
About You
To be successful in this role, you will bring:
- Current AHPRA registration as a Registered Nurse.
- At least 2 years’ clinical experience in aged care or case management.
- Experience in Home Care Packages is highly desirable.
- Strong knowledge of the Aged Care Standards, reforms, and consumer-directed care.
- Experience in assessment, service brokerage, and budget management.
- Excellent communication, problem-solving, and relationship-building skills.
- Ability to work both independently and as part of a team.
