Permanent Position | Relationship-Focused | Purpose-Driven Work
We’re seeking an experienced and compassionate Customer Service Coordinator to join our team at Interchange on a permanent basis. This is a pivotal role where you’ll support people with disabilities to live connected, valued, and empowered lives.
You'll take a relationship-based approach, working closely with customers, their families, support teams, and other stakeholders to ensure high-quality, person-centred support. A key focus will be on safeguarding, managing risk, and upholding quality standards.
In this role, you will:
Develop, implement, and review individualised support plans in collaboration with customers and their networks
Lead, mentor, and support a team of Disability Support Workers
Build strong, respectful relationships with customers, families, and allied health professionals
Ensure services align with NDIS practice standards and organisational values
Actively contribute to continuous improvement initiatives and service planning
Participate in the on-call roster, providing support outside of standard hours as required
What we’re looking for:
Experience in the disability, community, or health services sector
Strong understanding of the NDIS and person-centred approaches
Excellent communication and leadership skills
A commitment to inclusion, empowerment, and human rights
This role is based in our Belmont office and includes travel across the Perth region.

What do you need to apply?
Equivalent to VET Certificate IV in relevant studies and/or equivalent knowledge and experience obtained within the disability industry
Significant experience with NDIS plan utilization and funding arrangements.
Excellent written and verbal communication skills, with the demonstrated ability to build strong relationships with people with disabilities, colleagues and members of the community.
Ability to work autonomously and in a team
Thorough knowledge of relevant government legislation and standards
Excellent organisation and time management skills and an ability to seek creative solutions to overcome issues.
Proficient computer literacy
Current WA C class drivers license and own car.
Current valid car insurance that provides cover for business use
Current First aid/ CPR certificate (Obtained in the last 12 months)
NDIS Worker Screening Clearance
Working with children's Check (WWCC)
