About The Opportunity
The Care Partner is the primary contact for a Home Care Packages (HCP) care recipient or their family, responsible for the assessment of care needs, identification of care goals in collaboration with clients, development of care plans and co-ordination of service delivery to ensure expenditure is within budget. The Care Advisor ensures program activities are in accordance with the Aged Care Act, HCP program guidelines and the aged care quality standards.
The day-to-day responsibilities
- Establishes rapport with care recipients and their family/representatives.
- Manages a dedicated portfolio of clients, meeting all monthly, quarterly, and annual review requirements.
- Responds to deteriorating health by reviewing care needs and services.
- Identifies vulnerable clients and ensures their care plan responds to needs associated with vulnerability.
- Provides care recipients and their family members with information as required about:
- Home Care Package.
- Charter of Aged Care Rights and Home Care Package agreements.
- the establishment of their individual budget and monthly statements.
- How the funds can be spent.
- How to ensure these meet legislative compliance and accountability.
- Community resources that can be accessed for additional support.
- Participates in and/or initiates client case conferencing.
- Arranges and attends meetings with key stakeholders as required.
- Ensures a collaborative approach to care planning, ensuring all stakeholders are involved.
- Seeks clinical advisory support to ensure the clinical needs of the client are met and regularly reviewed.
- Participates in all training to meet requirements of the role as directed.
- Assists care recipients and families to make informed choices about their care through the provision of accurate and high-quality information.
- Undertakes regular review and assessment of plans with clients, adjusting as required and considering client preferences.
What success looks like for this role
- Aged care experience – including knowledge of the issues facing older people or people with disabilities and their carers
- Demonstrated understanding of aged care standards and best practice in community aged care
- Excellent communication, problem solving, influencing & negotiation skills, including a capacity to facilitate complaint resolutions
- Thorough understanding and ability to support compliance with Occupational Health & Safety, Equal Opportunity and other professional & legal requirements which affect day-to-day care and other services for clients
- Ability to work independently and as a team.
- Ability to prioritize and be flexible to attend to changing workplace demands
