The Role:

We are seeking a dynamic individual to support collaboration between Diabetes WA and primary health care providers by promoting integrated diabetes care, and engaging health professionals with Diabetes WA programs and services. Reporting to the Programs & Community Operations Manager, the role involves program coordination, relationship building and strategic outreach to enhance awareness and referral to diabetes programs across the state, and will require travel to regional and remote locations.

Position Details:

Type: Part-time (0.6 FTE), with flexible work options considered.

Salary: Competitive salary starting at $39.85 per hour, plus superannuation.

Key Responsibilities:

Effectively support and engage with leaders and health professionals within General Practise and other health services with the purpose of:

Increasing GP and primary care stakeholder knowledge, awareness and engagement with Diabetes WA.

Building awareness and understanding of the NDSS, and how to refer people to NDSS services and programs.

Developing relationships and opportunities for collaboration with Primary Healthcare Networks (PHN) and other health services.

Promoting training, education and quality improvement opportunities to develop diabetes related skills and knowledge in the primary care workforce.

Plan and support Health Professional events and information sessions, including attending and facilitating events as required.

Ideal Candidate (Selection Criteria):

Strong working knowledge of primary health care systems, services and networks, including current issues and strategic priorities.

Demonstrated ability to establish networks and develop effective working relationships with internal and external stakeholders.

Demonstrated ability to build effective relationships and communicate with health professionals and sector leaders.

Experience in promoting services to health professionals.

Well-developed event planning, stakeholder engagement, and program coordination skills.

Some flexibility is required for attending events during the evening and at weekends, as well as the availability for travel throughout metro and regional WA.
