Your Role
The Cancer Support Officer will provide practical day-to-day support and assistance for Aboriginal and Torres Strait Islander people/families and community navigation the health and community support systems involved in and around the cancer journey. This will involve stakeholder mapping, establishing referral pathways to support services and liaison with medical and non-medical staff.
· Improve patient management system functionalities for cancer data recording and enhance continuous quality improvement (CQI) activities
· Build partnerships with mainstream cancer services and organisations
· Advising the community through the promotion of initiatives in regard to cancer prevention and screening days or months within the community
· Liaise with external health care providers and provide case management support to a multidisciplinary team (including medical and therapeutic staff) to ensure the delivery of culturally-appropriate transitions with high quality to ensure continuous quality improvement
Promote allied health services and effective risk management.
Key Selection Criteria
· Encourage positive behaviour change in community members by planning, developing, and delivering health promotion activities using a variety of resources and strategies
· Assist those client/s and their families with diagnostic testing and the arrangement of appointment/s
· Foster and maintain relationships with other agencies to benefit clients and the organization.
· Report through an escalation process, any concerns that may place a client or their family at unacceptable risk.
Qualification/s:
Registered Nurse or Aboriginal Health Practitioner and a current practicing certificate of registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Other Screening Requirements:
Valid Drivers License that allows you to drive in Australia.
Victorian Employee Working with Children Check card.
National Police Check.
