Your Key Responsibilities
Deliver and lead Home Medicines Reviews (HMRs)
Coordinate and review the medication packing/DAA service and lead technician engagement
Provide chronic disease support – diabetes, cardiovascular, respiratory, mental health
Support and enhance our harm minimisation programs, including Opioid Replacement Therapy (ORT)
Collaborate with local GPs and allied health to build shared care models
Mentor and guide pharmacists and interns in professional services delivery
Option to utilise Scope of Practice accreditation for expanded clinical services (if applicable)
You’ll Be a Great Fit If You Have:
AHPRA pharmacist registration (essential)
HMR accreditation and provider number (essential)
CDE credentials or willingness to obtain (desirable)
Experience in DAA services, medication reviews, and patient follow-up
Vaccination accreditation + valid First Aid & CPR
Experience or interest in ORT and harm minimisation programs (preferred)
Strong communication skills and a patient-first, proactive mindset
