About the Role
Reporting directly to the Aged Care Services Manager, as the Clinical Care Manager you will be considered a subject matter expert for the Aged Care Services Team as well as the broader organisation, for all matters relating to clinical care quality, safety, compliance and risk management. You will also have direct leadership responsibility for a team of Home Care Package Facilitators, providing ongoing management and mentoring to ensure the delivery of safe, high-quality, and person-centred care to consumers.
Duties & Responsibilities
- Provide leadership and management to HCP Facilitators to ensure quality outcomes are met
- Provide clinical guidance and mentorship to care staff, ensuring best practices in clinical care are followed
- Lead the coordination of clinical needs assessments of HCP consumers to ensure needs are met and regularly reviewed
- Develop and maintain clinical care related policies, procedures, and frameworks as they relate to clinical standards and practice - contributing to quality improvement of systems and processes
- Actively contribute to the clinical governance framework and incident management systems by reviewing existing clinical risks, complaints and feedback, and providing relevant recommendations
- Prepare and maintain relevant reports for Senior Management Team and external funding bodies on a regular basis
- Create opportunities to provide education and training to relevant teams within the organisation
- Liaise with a range of internal and external stakeholders including consumers, families, and service providers
Skills & Experience required
- Demonstrated experience working as a Registered Nurse Level 3 (RN3) under the Nurses Award 2020
- Current AHPRA registration
- Proven abilities in policy development and business process / clinical practice improvement
- Ideally you will have experience in community nursing or a non-profit setting, although this is not essential - what is more important will be your fit for the values and culture of this organisation
- Someone who is proactive, self-motivated and solutions-focused
- Excellent attention to detail and a commitment to best practice
- High level of cultural and emotional intelligence
