An exciting opportunity exists for an experienced and forward-thinking Home Care Manager to lead the delivery of Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) services in the Echuca region.

This is a critical leadership role focused on delivering client-directed care, supporting reablement goals and preparing the organisation for the significant changes ahead as Australia transitions to the new Support at Home (SAH) program.

About the Role

As the Home Care Manager, you will be responsible for ensuring high-quality responsive care services that meet current aged care standards while also positioning the team and service model for a successful transition to SAH. You will lead a multidisciplinary team, champion person-centred care and drive outcomes aligned with client goals, wellbeing and independence.

Key Responsibilities

  • Manage and develop client services under HCP/CHSP with a consistent person-centred approach to care
  • Lead and support a multidisciplinary team to complete timely assessments and develop personalised care plans
  • Oversee referral pathways including to My Aged Care and relevant internal and external services
  • Ensure clients receive accurate information about services, rights, responsibilities and available supports
  • Prepare the service and team for upcoming Support at Home program changes ensuring alignment with policy direction, funding reform and care model requirements
  • Monitor client outcomes, satisfaction and wellbeing through structured review processes
  • Champion community participation, client feedback and program evaluation for continuous improvement
  • Maintain effective workforce planning, rostering and staff engagement to meet care needs and maintain service quality
  • Promote a strong workplace culture of safety, inclusion, trust and professional development

What You Bring

  • Extensive experience in managing HCP and/or CHSP programs in a community setting
  • Strong working knowledge of current aged care reform particularly the Support at Home (SAH) program design, timelines and implications for service delivery
  • Proven leadership and change management capabilities
  • In-depth understanding of person-centred care, reablement and wellness approaches
  • Demonstrated ability to lead high-performing teams and collaborate across disciplines
  • Solid understanding of aged care compliance, Aged Care Quality Standards and clinical governance frameworks
  • Excellent communication and stakeholder engagement skills
  • Alignment with a values-based care approach that prioritises dignity, choice and independence for older people
