The Targeted Care Package (TCP) Case Manager plays a critical role in supporting children and young people involved with the Child Protection system to transition into care arrangements that better meet their individual needs. These alternative placements may include returning to family, kinship care, foster care, independent or semi-independent living, or other tailored care settings.
The primary goal of TCPs is to improve outcomes for children and young people in out-of-home care. All care planning and decision-making is guided by the best interests of the young person, ensuring a flexible, individualised approach that responds to their unique circumstances and goals
Selection Criteria:
A relevant tertiary qualification such as Social Work, Welfare or Behavioural Science and/or proven experience relevant to this role.
Knowledge of the developmental and therapeutic needs of children and young people who have experienced trauma, abuse and neglect.
Demonstrated experience working with children, young people, and families experiencing complex needs, particularly those involved in the Child Protection or Out-of-Home Care systems.
An understanding of working within a case management framework to provide effective support to children and young people with multiple support needs.
