Key Responsibilities:
Conduct comprehensive assessments to understand clients’ needs across physical, psychological, social, and vocational areas.
Develop and implement personalised recovery plans in collaboration with clients, families, and healthcare professionals.
Coordinate a range of services including medical, allied health, rehabilitation, vocational, and community supports.
Advocate for client rights and needs, ensuring access to appropriate care and services.
Monitor and review client progress, adjusting plans as necessary, and maintain detailed documentation.
Manage a diverse caseload, prioritizing effectively to meet client goals.
Provide crisis management and support during challenging situations.
Build and maintain strong working relationships with medical professionals, service providers, and stakeholders.
About You:
Diploma or Bachelor’s degree in Social Work, Community Services or related field.
Experience in case management, rehabilitation, or a related role.
Strong client-centered approach with excellent interpersonal and communication skills.
Demonstrated problem-solving, negotiation, and advocacy abilities.
Organised and able to manage competing priorities efficiently.
Proficient with Microsoft Office Suite and client management systems.
Current Victorian Driver’s License and reliable vehicle.
Valid Working With Children Check and National Police Check.
