About the role

We are seeking an experienced and dedicated Home Care Package (HCP) Stay at Home (S@H) Case Manager to join our team at Ambition Health Group. This part-time role is based in Werribee, Victoria and offers the opportunity to make a meaningful impact on the lives of older adults and individuals with disabilities.

This is a permanent part-time position, starting at 3 days per week. As our service continues to grow nationally, this role comes with a guaranteed and secure pathway to increase to full-time hours, offering you unparalleled job security and career progression.

What you'll be doing

Develop and implement individualised care plans to support clients in remaining independent and living safely in their own homes

Coordinate and monitor the delivery of home care services, including personal care, nursing, allied health, and domestic assistance

Regularly review and adjust care plans to ensure they continue to meet clients' changing needs

Collaborate with clients, their families, and a multi-disciplinary team to provide holistic and person-centred care

Advocate for clients and liaise with healthcare providers, government agencies, and other community organisations

Maintain accurate and up-to-date client records and documentation

Contribute to the continuous improvement of our home care services

What we're looking for

Relevant tertiary qualification in aged care, disability support, or a related field

Minimum 2 years' experience as a Case Manager or in a similar role within the aged care or disability support sector

Demonstrated understanding of the Home Care Package program and the aged care and disability support industry

Excellent communication and interpersonal skills, with the ability to build trusting relationships with clients and their families

Strong problem-solving and critical thinking skills to identify and address clients' needs

Proficient in using computer systems and client management software

Commitment to providing high-quality, person-centred care
