About the Role:
The Youth Housing First program is a new, five-year initiative providing housing and wraparound support to eligible young people transitioning from residential care, other care settings, or youth justice. Delivered in partnership with MCM and Quantum Support Services, this innovative program focuses on long-term housing stability, strengthened community connections, and improved life outcomes.
The Housing First Case Manager plays a pivotal role in supporting young people with a care experience as they transition into independent living. This role is grounded in the principles of Advantaged Thinking, Housing First, Sustaining Tenancies, and Healing Informed Practice, placing the young person’s voice, autonomy, and potential at the centre of all support strategies. The Case Manager will deliver individualised, flexible, and relationship-based support to young people exiting residential care, alternative care settings, or custodial environments. By fostering strong, trusting relationships, the Case Manager works alongside each young person to co-design and implement tailored plans that respond to their unique needs, aspirations, and life circumstances.
Selection Criteria:
A relevant tertiary qualification such as Social Work, Welfare or Behavioural Science and/or proven experience relevant to this role.
Knowledge of the developmental and therapeutic needs of young people who have experienced trauma, abuse and neglect.
An ability to adequately assess young people’s needs, and develop, evaluate, and vary support plans in relation to those needs
An understanding of working within a case management framework to provide effective support to young people with complex needs.
