PURPOSE
The Support Coordinator plays a vital role in empowering NDIS participants to build the skills and confidence needed to navigate their supports and live more independently. The primary purpose of this role is to assist participants to understand, implement, and optimise their NDIS plans by connecting them with a mix of funded supports, informal networks, community services, and mainstream systems.
Support Coordination is a time-limited service that focuses on strengthening the participant’s capacity to manage their supports now and into the future. This includes resolving barriers, promoting choice and control, and helping participants become active and connected members of their community.
Through person-centred and goal-oriented support, the Support Coordinator ensures that services are effectively integrated and aligned with the participant’s needs, values, and aspirations—supporting the broader vision and values of the organisation.
SPECIFIC ACCOUNTABILITIES
Assist in the growth and expansion of NEWCHAPTER services, including attending, facilitating, and completing the onboarding process (quotes, service agreements, onboarding assessment, risk management and all other associated documentation).
Support participants to understand, utilise and implement their NDIS Plans.
Identify risks or crises that may impact the participant’s health, safety or plan outcomes, and take appropriate action, including escalation to appropriate internal or external stakeholders as needed (e.g., guardians, safeguarding bodies, crisis services, NDIA).
Assist participants in preparing for plan reviews by gathering evidence, progress reports, and outcome data. Provide written summaries or reports as required by NDIA to support plan reassessments.
Coordinate and facilitate access to various support services to enable best supported outcomes, including but not limited to providers, allied health, informal / formal community, and mainstream services.
Support participants to build capacity to address barriers in accessing service provisions.
Advocate on behalf of participants when conflicts arise with service providers or when navigating complex systems, ensuring that participants' voices, rights, and preferences are upheld.
Maintain accurate documentation and records (working log / billable record) of all applicable support-related activities and progress within New Chapters document management systems, one drive + CMS (Lumary).
Build effective relationships with participants and their families/persons responsible while maintaining appropriate and professional boundaries, ensuing regular contact is maintained.
Stay updated on relevant policies, regulations, and best practices in the field of support coordination.
Ensure regulatory reporting and organisational compliance requirements are adhered to.
Effectively work with other organisations to provide the best outcome for participants and their families/persons responsible and the most efficient use of community resources and funding.
Identify areas of improvement and actively seek participant feedback and address gaps/issues that impact negatively on participants, goal outcomes and/or staff.
Undertake regular assessments of existing and new participants to ensure best practice.
Ensure regular contact is maintained and recorded.
SELECTION CRITERIA
Tertiary qualification or diploma, or substantial experience and study to demonstrate competence sufficient to perform the duties required of this position.
Demonstrated understanding of NDIS rules, NDIS Practice Standards, NDIS Code of Conduct, NDIS Price Guide and NDIS Support Catalogue.
Strong interpersonal skills including the ability to communicate effectively with a range of people and in a variety of situations.
Demonstrated capacity to manage budgets and ensure effective use of funding to achieve the best outcomes.
Demonstrated organisational, planning, high level to detail and problem-solving skills.
Ability to work effectively in a multi-skilled environment which is subject to work pressures and change.
Experience with producing high quality and effective communication (written and oral).
Computer skills including experience with the Microsoft Office, and the ability to adapt to new in-house systems.
Current drivers’ licence; and
Current Working with Vulnerable People Registration with NDIS Endorsement.
Undergo NDIS Worker Screening Check.
