Yoorana Gunya Family Healing Centre Aboriginal Corporation is an Aboriginal Community Controlled Health Organisation (ACCHO) based in Forbes, NSW. YG provides holistic, culturally appropriate, comprehensive primary health care services, mental health support, social support services and allied health to Aboriginal and non-Indigenous people. YG provides a range of programs to enhance Aboriginal and Torres Strait Islander people’s overall wellbeing including health related programs, programs to strength community connection, mental health awareness and more.
The YG clinic is supported by General Practitioners, Nurses, Aboriginal Health Practitioners, Community Support Team including Social and Emotional Wellbeing, Family Violence, Integrated Team Care and various visiting allied health services including Optometrist, Audiologist, Cardiologist, Podiatrist, Dietitian, Gastroenterologist. YG has visiting Counsellor, Mental Health Nurse, Psychologist and refers to Psychiatry.
About the role
The purpose of the Cancer Health Promotion Officer is to create awareness and deliver education to community members to encourage positive behaviour change by planning, developing, and delivering health promotion activities using a variety of resources and strategies. These programs and resources are to be locally relevant and culturally safe and responsive with the intention of increasing the adoption of health promoting behaviours and screening participation in Aboriginal and Torres Strait Islander community. Provide practical day-to-day support and assistance with navigating the health and community support systems involved in and around the cancer journey.
The role will work closely with General Practitioners and clinic nurses in both AMS and mainstream General Practice, Local Health District and community. The position will focus on strengthening the partnership and Aboriginal community engagement to increase cancer screening participation to ensure early diagnosis and increased chance of survival. Working closely with the YG clinic, community support team, mental health support team and other professionals to improve the health and wellbeing of Cancer patients.
The position will be required to:
- Develop local cancer communication campaigns and engagement activities
- Create and deliver cancer awareness messaging
- Develop a holistic screening model of care
- Develop health promotion resources and materials
- Improve delivery and accessibility of mainstream cancer services and provide support for community members with cancer
Qualifications and Experience
• Tertiary qualifications in Health Promotion, Pulblic Health, Nursing, or related field.
• Understanding of the diverse needs of Aboriginal people and cultural appropriate care.
• Demonstrated knowledge, skills and experience working in cancer prevention and screening programs.
• Strong communication and facilitation skills
• Ability to work independently and as a part of a multidisciplinary team
• Preferred confirmation of Aboriginality
Primary Responsibilities and Duties - The role is focused on prevention, screening, early diagnosis, treatment and supportive care
Prevention
• Develop local cancer communication campaigns and engagement activities
• Create a deliver cancer awareness messaging
Screening
• Upskilling clinical staff on National Cancer Screening Register and screening programs
• Develop a holistic screening model of care
• Assist the Yoorana Gunya SEWB team in providing programs to the Aboriginal community and raising awareness about Cancer Screening.
Early Diagnosis
• Health promotion campaigns focused on health seeking behaviour
• Develop health promotion resources and materials
• Ensure appropriate comprehensive, multidisciplinary primary health care is provided to all patients with a focus on cancer patients to support them and their families. This will be ensured by the following: completed Aboriginal health assessments (715’s), Chronic Disease Management and general consultations completed with onsite General Practitioners, Registered Nurses, Aboriginal Health Practitioners, any other allied health or mental health support.
Supportive Care
• Work closely and develop care plans for cancer patients, undertake assessments of client’s mental health wellness to determine their individual needs and risk level.
