We are BCR Communities – a not-for-profit award winning community organisation serving communities within the Illawarra and Shoalhaven for over 30 years (formerly Bay & Basin Community Resources). We are seeking to employ an experienced Registered Nurse to join our growing clinical team providing person centred clinical care and support for clients receiving Home Care Packages (HCP) across the Illawarra and Shoalhaven. Based in Nowra, this role is available on a part time basis, approximately 38 hours per fortnight rostered between Monday and Friday, no evenings or weekends!
You will be supporting our older clients to support healthy ageing and wellness goals in their care plans, in accordance with the Aged Care Quality Standards. Your role will provide clinical support for consumers including, but not limited to wound care, clinical reviews and behaviour support. You will achieve this through working in a multidisciplinary team, forming strong professional relationships with clients and their carers, whilst working closely with Care Managers and all health professionals involved in their care.
We seek individuals who:
Have current AHPRA registration and has a special interest to work with older Australians
Can provide clinical expertise and knowledge for both our clients and home care package team facilitating early identification and intervention for signs of deterioration
Partner with other health care team members in a solution focused manner to achieve client goals
Display a passion for delivery of a restorative wellness model, which aims at improving the quality of life for older Australians
Are committed to building a positive workplace culture reflecting our vision, mission and values
Possess computer skills, including MS Word, Excel, Outlook, and client management systems
Have received a primary dose of COVID 19 vaccination
