We are currently seeking a Part-Time Registered Nurse to join our team, providing comprehensive assessments for our clients ensuring high standards of clinical care are met in line with the Aged Care Quality Standards and NDIS requirements.
Position Overview:
The Registered Nurse (RN) will be responsible for conducting comprehensive client assessments, including falls risk assessments and the completion of RUDAS (Rowland Universal Dementia Assessment Scale) when applicable. The RN will ensure that clients' care needs are met in a timely and professional manner, in alignment with best practice standards and compliance with Aged Care Quality Standards. This role plays a crucial part in the clinical care team, contributing to the development of individualized care plans and ensuring the quality of care provided aligns with available funding and client needs.
Key Responsibilities:
Client Assessments:
Conduct assessments of clients in their home environment to identify their clinical needs, including falls risk assessments and RUDAS where appropriate.
Clinical Documentation:
Maintain accurate and timely records of client assessments, care plans, and any clinical observations in line with legislative requirements and internal policies.
Client Monitoring:
Regularly monitor the health status of clients, ensuring any changes are promptly identified and addressed with appropriate interventions.
Falls Risk Management:
Complete and review falls risk assessments for clients, implementing and recommending preventive measures to reduce risk and improve overall safety.
Health Education & Support:
Provide education to clients and their families on health conditions, medications, and best practices to manage care needs at home.
Collaboration:
Work collaboratively with other healthcare professionals, including care managers, allied health staff, and external healthcare providers, to ensure integrated and holistic care.
Compliance & Quality Assurance:
Ensure all clinical activities are in compliance with the Aged Care Quality Standards, the Guidelines for Home Care Packages, and relevant legislation.
Clinical Supervision:
Provide clinical guidance and support to less experienced staff and assist in the training of new team members in clinical care procedures.
Incident Reporting & Follow-up:
Participate in incident reporting systems and ensure follow-up actions are completed, especially for incidents related to falls or clinical concerns.
Reassessment After Hospital Discharge:
Conduct a comprehensive reassessment of clients following hospital discharge to evaluate their current health status, adjust care plans, and ensure appropriate follow-up care is in place.
Emergency Response and Crisis Management:
Lead the response to any medical emergencies or critical incidents. Ensure timely interventions and proper documentation of outcomes.
Qualifications:
Education:
bachelor’s degree in nursing and current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Experience:
Minimum of 2 years' clinical experience as a Registered Nurse, preferably in home care, aged care, or community health settings.
Certifications:
Current First Aid and CPR certification.
Other:
Valid driver's license and access to a vehicle (if required for client visits).

Clinical Knowledge:
Sound understanding of clinical assessments, falls risk management, and dementia care principles, including the use of tools like the RUDAS.
Communication:
Strong communication skills, with the ability to interact effectively with clients, families, and multidisciplinary teams.
Attention to Detail:
High level of attention to detail in clinical assessments, documentation, and care planning.
Clinical Judgment:
Ability to make sound clinical decisions and prioritize care based on client needs.
Organizational Skills:
Ability to manage multiple client assessments and prioritize tasks in a fast-paced environment.
Empathy & Compassion:
Strong interpersonal skills, demonstrating empathy and understanding when working with vulnerable clients.
Knowledge of Regulations:
Familiarity with the Aged Care Quality Standards, Home Care Packages Guidelines, and relevant legislation affecting the home care and aged care sectors.
Technology Proficiency:
Competency in using electronic health records and other relevant software for documentation and communication.
Language Skills:
Fluency in English is required. Proficiency in Arabic will be considered an advantage.
Position Type: Part-Time
Work Hours: 2 days a week, 7 hours per day. Days may be flexible depending on availability, with options for Monday & Tuesday or Wednesday & Thursday.
Hourly Rate: $45 per hour
