About the role

We are seeking a dedicated and compassionate Support Coordinator to join our team at Human Core Pty Ltd. As a part-time Support Coordinator, you will be responsible for providing personalised support and coordination services to clients in the south east Brisbane, Logan Central QLD 4114 area. Your role will be crucial in helping our clients access the services and resources they need to live independently and improve their overall wellbeing.

What you'll be doing

Assessing client needs and developing tailored support plans to address their individual goals and requirements

Coordinating and facilitating access to a range of support services, including healthcare, social services, and community programs

Providing emotional support and guidance to clients, helping them navigate the complexities of the support system

Collaborating with a multidisciplinary team of professionals to ensure clients receive comprehensive and coordinated care

Maintaining detailed client records and documentation to ensure continuity of care

Advocating for client needs and acting as a liaison between clients and service providers

Participating in regular team meetings and professional development activities to enhance your skills and knowledge

What we're looking for

A tertiary qualification in Psychology, Counselling, Social Work, or a related field

Proven experience in a similar Support Coordinator or Case Management role, preferably in the healthcare or social services sector

Excellent communication and interpersonal skills, with the ability to build trusting relationships with clients

Strong problem-solving and critical thinking skills, with the ability to navigate complex support systems

Demonstrated empathy and understanding of the challenges faced by individuals with diverse needs and backgrounds

Proficiency in using relevant technologies and databases to manage client information and coordinate services

A genuine commitment to promoting the independence, dignity, and wellbeing of our clients
