We are looking for Support Workers for multiple roles in Hornsby / surrounding areas, including:

Hornsby

• Male Support Worker required for a male participant
• Participant has fractures in both hands
• Requiring 2 hours' assistance daily (Monday to Sunday) starting from 8AM or 9AM
• Mainly assisting with personal care

Berowra Heights

• Male Support Worker required for male 16-year-old participant
• Approximately 4-5 hours per week (Monday - Wednesday from 4PM - 5:30PM)
• You will still be paid for 2 hours for shifts which are under the minimum 2-hour shifts
• Type of support: Social engagement, building rapport, exploring participant’s interests, encouragement with social outings and maintaining comfort during car travel (he has recently made progress overcoming car-related anxiety)

Hornsby Casual Pool

• Support Workers to join our NSW Flexible Response Team
• You will work with different participants around Sydney
• This is an on-call/casual pool role with NO guaranteed hours weekly
• Shifts are a minimum of 2 hours
  
  

Dural

• Support Worker required for 39-year-old male who is blind
• Some days needs dialysis, so service will require taking him from his home in Dural to his appointment, wait while in dialysis, then transporting him home
• Excellent English communication skills required
• Support required on Tuesday, Thursday and Saturdays from 6:30am (4-5 hours each shift)

Requirements:

• Valid Driver's Licence & access to a comprehensive-insured car
• NDIS Worker Screening Check (NDISWC)
• Working with Children Check (WWCC) for paid employment
• Current First Aid & CPR certification (or willingness to obtain)
• Must be willing to provide all aspects of support including anything from meal preparation to domestic assistance, community access and personal care
• Experience with Disability and/or Aged Care

What you'll do:

• Complete all tasks relevant to support work including anything from meal preparation to domestic assistance, community access and personal care
• Empower participants to achieve their goals and live as independently as possible
• Be a trusted and reliable point of contact for participants and their families
• Maintain detailed and accurate documentation such as shift notes, progress notes, behaviour charts and incident reports.
• Follow company policies and procedures
