About the Role
We are seeking a passionate and experienced Service Coordinator to provide day-to-day leadership and direction to teams of Person-Centred Support Workers who support people with complex psychosocial and other disabilities. In this role, you will play a central part in driving meaningful impact by mentoring and inspiring frontline staff to deliver high-quality, responsive services.
Working under the guidance of the Operations Manager, and in collaboration with participants, families, guardians, and other key stakeholders, you will help co-design and implement therapeutic, person-centred supports. You will also contribute to service evaluation to ensure effectiveness and positive participant outcomes.
Based in Edwardstown, SA, this role requires regular home visits to support frontline teams, monitor service quality, check participant wellbeing and safety, and complete site audits.
Key Responsibilities
Lead person centred active support within the CLO Therapeutic Model of Supports, ensuring that participants are at the centre of everything we do, with their human rights and choice and control protected.
Ensure the ongoing review and currency of participant plans.
Work collaboratively with participant support teams to ensure high quality, participant driven outcomes.
Monitor and assess plan implementation to ensure high quality, participant driven outcomes.
Ensure that all supports are properly documented, and that all recording meets the requirements of defensible documentation.
Lead the development of Person-Centred Active Support Workers by providing regular coaching, mentoring and role modelling, as well as formal supervisions and the development Personal Development Plans for all staff.
Design innovative, person-centred rosters which can be delivered within participant funding.
Forward plan rosters through effective workforce and risk management practices.
Review incidents and support internal and external investigation and reporting requirements.
Build and maintain strong relationships with participants, staff and key stakeholders.
Provide relevant reporting both internally and externally when required.
Required Skills and Experience
Have experience with and a passion for working with people with psychosocial disability to achieve quality of life outcomes.
Have experience in the effective implementation of plans relating to the support of people with psychosocial disability.
Have experience of and a passion for developing staff so that they can work confidently with people with psychosocial disability.
Be flexible and able to adapt quickly to changing situations, whilst prioritising your work effectively.
Be a good written and verbal communicator, able to develop and maintain working strong relationships.
Have attention to detail, ensuring that documentation and reporting requirements are met.
Understand mental health/hospital discharge planning processes and needs.
Advanced understanding of Microsoft applications.
Qualification and Essential Licences
Current Driver’s Licence
NDIS Worker Screening Check (or willingness to obtain)
Working with Children Check (or willingness to obtain)
Valid First Aid and CPR certificate
Tertiary qualification in Disability, Social Work, Behavioural Science, Psychology, or a similar/equivalent field
Must meet Australian visa requirements and have the legal right to work in Australia
