We’re on the lookout for experienced Support Worker’s with a strong background in mental health and the Recovery Model to join our community-based team.
In this role, you will be out and about, supporting clients in their homes and helping them connect with their local community. Whether it’s a trip to the shops, a walk in the park, seeing the latest movie, or simply grabbing a coffee — no two days are the same and you will adapt with client choice.
You’ll be working closely with clients on the ICLS program and NDIS, helping them chase their goals, build independence, and boost their confidence. You’ll also be liaising with families, carers, and other key players to make sure everyone’s on the same page.
This opportunity is part-time within Monday to Friday, between 9:00am and 5:30pm (15 – 35 hours per week depending on your availability). Your offered shifts will be set, no rotating or changing rosters. Rise clients are spread across East and North Metro (some South) clients. We are seeking applicants are happy to travel across all Metro areas.

Relevant qualification e.g., Disability, Community Services
Experience and solid understanding of recovery-oriented practice
Compassionate, reliable and committed to person-centred support
Driving licence with use of a reliable vehicle for work purposes

Appointments to this position require a NDIS worker screening check. Costs for new checks will be covered and/or reimbursed by Rise.
