About the Role

You’ll provide occupational therapy services in the community across a mixed caseload including Home Care Packages (HCP), NDIS, Commonwealth Home Support Program (CHSP), Interim Hospital Packages (IHP), and Department of Veterans’ Affairs (DVA) clients.

Work includes home visits, equipment prescription, minor and major home modifications, reablement-focused therapy, and collaborating with families, GPs and care coordinators.

What You’ll Be Doing

Conducting functional assessments in clients’ homes

Developing tailored therapy and reablement plans

Recommending assistive technology and home modifications

Collaborating with families and other health professionals

Maintaining accurate, timely clinical documentation

Contributing to internal education and quality initiatives

What You’ll Bring

AHPRA registration as an Occupational Therapist

2+ years’ experience in aged care or community settings

Confident with AT prescription, home modifications, and rehab planning

Understanding of wellness, reablement, and restorative models

Excellent clinical reasoning and communication skills

Driver’s licence, reliable car, NDIS Worker Screening and WWCC
