Occupational Therapist - Aged Care | Applecross WA
$100,000 - $120,000 + Super | South of the River Caseload

We are seeking a dedicated and experienced Occupational Therapist to join a client-focused, supportive allied health team based in Applecross. This role is ideal for someone passionate about supporting older Australians to maintain independence and quality of life at home.

About the Role
You will provide community-based occupational therapy services to Home Care Package (HCP) clients. Your work will focus on enhancing daily function, safety, and wellbeing through tailored therapy plans, assistive equipment, and environmental modifications.

Key Responsibilities

  • Conduct person-centered functional assessments in client homes
  • Develop and review individualised therapy programs
  • Recommend assistive technology, equipment, and home modifications
  • Liaise with families and care teams to support goal achievement
  • Maintain accurate and timely clinical documentation
  • Contribute to internal education, training, and quality improvement

What You'll Bring

  • AHPRA registration as an Occupational Therapist
  • 1+ years' experience in aged care or community OT
  • Experience prescribing AT, home mods, and rehab plans
  • Knowledge of wellness, reablement, and restorative models
  • Strong communication and clinical problem-solving skills
  • Driver's Licence, reliable car, NDIS Worker Screening Check & WWCC
