Case Manager:
Opening and closing files (physical and electronic files) including issuing of intake forms, liaising with participants, carers and families to complete all intake forms and monitoring their return.
Preparing individualised support plans.
Researching service providers and completion of referrals.
Plan/review/approve participants invoices, funding and budgets.
Reviewing provider service agreements to ensure they are reasonable and work within budgets.
Build rapport, negotiate, develop and/or maintain mutually beneficial relationships with a diverse range of stakeholders, Local Area Coordinators or other NDIS Providers to achieve organisational and participant objectives.
Consult with participants, carers, and families to facilitate service planning.
Organise and book in meetings.
Answer and direct phone calls.
Filing.
Data entry and management of Careview.
Drafting of reports and documentation.
Use and update software system and technology for wellbeing updates.
Assist with company auditing.
Minimum requirements are:
Knowledge & Experience of NDIS
Computer Skills
Monday-Friday Availability 8.30am - 4pm
NDIS worker screening card (yellow card)
WWC card (blue card)
National Police Check (recent)
