About the Role
This office-based leadership role is part of the Fortify Health Care Services team and focuses on supporting clients with complex health needs to achieve improved health and wellbeing outcomes. We are seeking a Registered Nurse, Enrolled Nurse, or an experienced Community Worker to step into this Case Manager position. In this role, you will provide clinical leadership, guidance, and support to a multidisciplinary team of Enrolled Nurses, Registered Nurses, and Support Workers, ensuring the safe and effective delivery of community disability support services to our clients.
This office-based and community based leadership role sits within the Fortify Care Service team, supporting clients with complex needs to achieve positive health and wellbeing outcomes. As a Case Manager, you will lead by example, providing training, supervision, and support to the care team while ensuring the delivery of impactful community nursing supports.
While predominantly office-based during Monday to Friday business hours, occasional weekend, PM, and ND shifts will be required. You will also participate in an on-call roster and cover shifts when required to maintain continuity of care.
This is a permanent role, offering a minimum 30 hours per week with shifts available up to 38 hours per week.
Key Responsibilities
Develop, modify, and evaluate care plans aligned with clients’ health needs and personal goals.
Provide clinical leadership, guidance, and supervision to the care team.
Monitor and evaluate the effectiveness of nursing care plan implementation.
Deliver training, supervision, and support to ensure care is delivered within scope.
Conduct home visits to review care plan effectiveness, support the team, and strengthen client relationships.
Perform clinical and risk assessments based on clients’ health needs and goals.
Oversee medication management.
Collaborate with GPs, care coordinators, and allied health professionals to ensure seamless care delivery.
Participate in a clinical on-call roster to provide advice and guidance to staff.
Cover shifts as needed to maintain continuity of care.
Ensure compliance with accreditation and clinical standards.

About You
Bachelor of Nursing with AHPRA registration (no restrictions) or similar qualifications
Previous case management experience required
Minimum 12 months as a case manager
Proven leadership and team-building skills.
First Aid/CPR Certificate, Blue Card, and NDIS Worker Screening (or willingness to obtain).
Strong administration, communication, and time management skills.
Driver’s license and own vehicle.
