Job Description:
We are seeking a compassionate and experienced Care Manager to join our team.
Your key responsibilities will include leading our direct care team and developing and overseeing individualised care plans that put the resident at the center of decision making all while complying with aged care regulations.
You will also be required to submit periodic mandatory reporting and internal management reporting.
Key Responsibilities:
Care Services: Coordinate and monitor the delivery to ensure they align with care plans and regulatory requirements. This includes AN-ACC assessments and medical appointments.
Positive Relationship: Foster between residents, families, and the multidisciplinary care team, addressing their concerns and preferences.
Legislative Knowledge: Maintain current knowledge of legislated and best practice requirements as well as industry trends and changes.
Work Environment: Promote a collaborative and positive work environment within the nursing and carers department fostering a positive work culture and ensuring high-quality care delivery.
Quality Assurance: Mandatory reporting and participating in quality assurance activities, including audits, inspections, accreditation, and quality improvement initiatives.
Assessment & Care Planning: Ensure that these meet the requirements for comprehensive person-centered care. Auditing care plans and ensuring the Registered Nurses understand the requirements for good care planning.
Weekly GP Clinic: Oversee and working with the Doctor and Allied Health to ensure positive outcomes and good health management of the residents. Follow up on all outcomes from doctor's visits and ensure families are kept informed of these outcomes.
Management of the Incident Management System: Review incidents, hazards and near misses in the clinical area. Ensure any impact is minimised with appropriate actions taken to improve systems and work practices. Maintain the safety of residents and clinical staff.
Supervision of Clinical Staff: Support and guide nursing staff and personal carers, ensuring tasks are completed effectively and in line with standards.
Aged Care Standards for Clinical Staff: Understand the requirements for compliance with and ensure that compliance is achieved. Participate in continuous quality improvement activities.
Conduct comprehensive assessments of residents’ physical, emotional, and social needs.
Develop and review individualised care plans in collaboration with residents, families, and healthcare professionals.
Inclusion in sub-committees and advisory bodies such as the Quality Care Advisory Body (QCAB) and Medication Advisory Committee (MAC).
Maintain current knowledge of legislated and best practice requirements as well as industry trends and changes.
Monthly Management and Board reporting.
Provide leadership for the clinical team in collaboration with the Director of Nursing/Facility Manager fostering a positive work culture and ensuring high-quality care delivery.
Relieve the Director of Nursing/Facility Manager as required, including on–call and scheduled leave.
Qualifications and Experience:
Bachelor of Nursing with current AHPRA registration
Current First Aid & CPR
National Disability Worker Screening Card (NDIS) (or willingness to obtain).
Experience in Care Management or a similar role.
Strong Clinical assessment and care planning skills.
Excellent communication, leadership, and people skills.
Knowledge of aged care regulations and accreditation standards.
Experience with LeeCare, Proda and MyAgedCare Portal is preferable.
