We are looking for an experienced leader to join Bellarine Community Health (BCH) as the Coordinator of our Social Support program. We are seeking an enthusiastic person with experience in the provision of Social Support (or similar) to guide an established team in providing program services to clients on the Bellarine. This role is offered part time, FTE 0.8 (4 days) and is based on the Bellarine Peninsula.
About the team
The Social Support program offers a range of activities that promote independent living and positive ageing that are under-pinned by client empowerment. The program provides clients an opportunity for social, physical, recreational, artistic & emotional interaction and is committed to a model of contemporary/best practice that is responsive to the changing needs of the local community.
About the role
The Coordinator, Social Support is responsible for the oversight of the Social Support team who work in partnership with Carers / Registered Supporters to optimise participation in Social Support Programs for suitable clients. This includes providing timely and comprehensive assessments focused on the social and well-being needs thereby optimising social connection in accordance with their goals and preferences.
The successful applicant will be someone with well-developed communication skills and a keen sense of community who is seeking an exciting and stimulating new opportunity. Ideally, they will have significant experience in the provision of Social and Community Support programs to specific clientele. They will also be familiar with managing and co ordinating staff, service development and an understanding of services targets
Some of the services this position oversees include:
Group based Social Support Services (Point Lonsdale and Portarlington sites)
Liaison with key stakeholders and local community groups
Transport of clients to and from provided programs
This well supported position will also be engaged in the planning for, and roll out of further group based and client transport activities related to the current service
We would like to hear from you, if you have…
A relevant health-related qualification or equivalent minimum 2 years’ experience in a similar role.
Demonstrated ability to creatively problem solve complex care situations.
An innovative and dynamic approach to the provision of social support services designed to optimise client engagement and connection with community.
Demonstrated understanding of the benefits of social connection and the concept of Social Prescribing.
Demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and users.
Ability to build rapport and trust with clients and colleagues, and excellent communications skills, both written and verbal.
Strong time management skills and ability to prioritise competing workloads and tasks.
Strong interpersonal skills with an ability to build positive relationships and work in a team environment and autonomously when required.
Demonstrated understanding of the current government funding streams for Social Support Services such as CHSP and Support at Home.
