Position Overview:
We are seeking a compassionate and dedicated Family Support Worker to join our team, with a special focus on supporting families of older adults living with dementia. In this pivotal role, you will provide emotional, social, and practical support to clients and their loved ones, helping them manage the complex challenges associated with ageing and cognitive decline. The ideal candidate will design and deliver personalised support programs, monitor progress, and work closely with other care professionals to ensure family members feel heard, informed, and supported throughout their journey.

We are especially interested in candidates who have experience working with clients from culturally and linguistically diverse (CALD) backgrounds, and who demonstrate cultural sensitivity and an inclusive approach. Proficiency in Polish or another Eastern European language is highly desirable. A proven track record of working with dementia clients and their families, particularly from CALD communities, will be highly regarded.

Duties and Responsibilities:

Assess family and client needs, particularly around dementia care, and design tailored support and education programs.

Conduct empathetic interviews with clients and family members to better understand their situation and provide appropriate guidance.

Monitor and report on client and family progress, updating care plans and liaising with the Supervisor.

Connect families to relevant external supports, including counselling, respite services, dementia-specific resources, and financial assistance.

Liaise with aged care service providers, community groups, and allied health professionals to ensure wrap-around support.

Support carers in managing the emotional and psychological impacts of caring for a person with dementia.

Educate families on effective communication strategies, behaviour management, and coping techniques for dementia-related challenges.

Assist in identifying and addressing broader community needs affecting older adults and their families.

Prepare accurate documentation, progress notes, and funding submissions as required.

Skills and Qualification Requirements:

AQF Certificate III with at least two years of on-the-job training, or AQF Certificate IV in a relevant field

Minimum 12 months of experience in a family support or aged care-related role

Demonstrated understanding of the impacts of dementia on individuals and families

Experience supporting clients and families from CALD backgrounds

Proficiency in Polish or another Eastern European language (highly desirable)

Strong interpersonal and problem-solving skills, with an empathetic and non-judgmental approach

High level of verbal and written communication skills

Ability to work collaboratively within a multidisciplinary team
