The Role
Connection Support Worker – Exit, plays a fundamental role within the Connection and Belonging supporting clients as they transition out of the settlement program and into independent living within the broader community. Working within the Connection and Belonging Settlement Team, the Connection Support Worker – Exit provides support facilitating the exit review assessment, exiting interviews and support to ensure a smooth and successful transition for clients who are assessed as ready to exit the HSP program. This position focuses on ensuring clients are exited from the program in a timely manner with ensuring clients have achieved self-sufficiency, have strong connections and a sense of belonging to their community so they can continue integration journey beyond the program's services.

What You’ll Be Doing:
Plan and coordinate the transition of clients exiting the HSP program including updating relevant internal systems and consolidating client data to support the team to plan exits.
Working with closely with the Audit and Review Team, Connection and Belonging team in finalizing exits to meet set KPIs.
Assess clients' readiness for independent living, identifying strengths, challenges, and areas requiring additional support or resources referring to SIS program or return to Connection and Belonging Team if further support required
Facilitate a smooth transition for clients from the settlement program to community-based services, maintaining continuity of care and support throughout the exit process.
Provide emotional support and encouragement to clients as they navigate the challenges of independent living, fostering resilience and self-efficacy.
Conduct exit interviews with clients to gather feedback on their experiences with the settlement program, identify areas of strength and improvement, and assess outcomes achieved.
In line with MA policy and procedures, mentor, guide and collaborate with volunteers, Bicultural Support workers, Causal Support Workers and student placements to provide direct case management services to specific clients and their families.
Take leadership, accountability and responsibility for meeting contractual Key Performance Indicators and ensure all supports comply with Multicultural Australia and program specific contractual, audit and service standard requirements.
Ensure the timely input of client related documentation and data entry, maintaining accurate and timely records in Multicultural Australia’s client case management system (CS Net) and within relevant government data management systems including submission of claims within a strict timeframe.
Other duties as directed by management.
Qualifications and Skills Required:
Possession of qualifications in Diploma or Certificate of the field of Social and Community Services;
Other relevant qualifications (including overseas qualifications) or experience may be recognised
Valid Queensland Driver’s Licence.
Possession of, or willingness and eligibility to obtain a Blue Card Working with Children (Risk Management and Screening) Act 2000 and the Working with Children (Risk Management and Screening) Regulation 2020, and
Additional Desirable Skills:
Experience in a not-for-profit organisation will be highly regarded.
Written and verbal communication skills in a language other than English.
Successful candidate will be required to have a valid driver’s licence, police check and apply for a Working with Children Blue Card.
