Our Streetlink program supports people experiencing homelessness with integrated, trauma-informed primary health services. In this part-time (38 hrs/fortnight) 12 months fixed term role as a Clinical Relationships Manager, you’ll lead community-based nursing initiatives and collaborate with a multidisciplinary team to improve health outcomes for vulnerable individuals and families.

What you’ll do:
Provide clinical nursing care including immunisations, chronic condition management, mental health and AOD support, and sexual health services
Coordinate outreach and in-reach health services to meet people where they are
Establish and maintain meaningful partnerships with GPs, community clinics, local councils, and homelessness sector organisations
Empower clients to take control of their health and connect with other services
Maintain accurate, high-quality patient records and comply with best practices in primary care delivery
What you’ll bring:
Registration with AHPRA as a Registered Nurse
Professional experience in community or primary health nursing
Strong relationship-building skills with the ability to engage diverse populations
A sound understanding of trauma-informed and person-centred care
Excellent communication, organisational, and problem-solving skills
Current Driver’s License, Police Clearance, and Working with Children Check
Desirable:
Mental health, paediatric, or midwifery qualifications
Experience working in GP clinics, with homeless communities, or in care coordination roles
