About the role:
Working in the Metro Northern region of Melbourne you will be based at our Northcote site. In this role, you will be an advocate for clients in facilitating discussions on identifying our client's goals and developing their care plans. This is a full time role working Monday - Friday, 9am - 5pm.
Duties include:
- Undertaking initial assessments of prospective clients and assist in making recommendations on capacity
- Assess, monitor, review, reassess the needs of clients within a holistic framework using a restorative and wellness approach
- Promote a harmonious team approach by acting as a mentor and supervisor to the Home Care Workers to ensure delivery of community services
- Facilitate effective client pathway support through case discussions and case conferences, review of feedback including incidents, hazards and handovers and monitoring of quality indicators
- Conduct annual reviews with clients, including, revising their care plan, goals, services and budgets to support our clients in living life to fullest
About you:
You are passionate about what you do and want to make a difference in the lives of our clients, enhancing their self-esteem and empowering them to make choices.
- Experience in safety in the Aged Care or Health Sectors
- Experience and expertise in in the provision of services in the area of Aged and Community services preferred
- Experience and expertise in working in a Clinical or Allied Health setting
- Case Management experience
- Experience in undertaking assessments, care planning, implementation, evaluation and documentation of quality client care with a person centred approach and in line with Consumer Directed Care principles
- The ability to communicate with staff, consumers, service providers, treating health professionals and other community members to ensure the development of needs based, consumer focused services delivery
- Thorough understanding and ability to support compliance with Safety, Equal Opportunity and other professional and legal requirements which affect day to day care and other services for clients
- Thrive in a team and also appreciate the one to one interactions

Essentials for working in Aged Care:
- A current Working With Children Check or willingness to obtain one
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance
