Caring Support @ Home are a Home and Community Care company offering a range of services to both aged and disabled clients, assisting them to remain in their own homes and maintain their independence.
We are currently recruiting an experienced PT/FT Clinical Care Manager to join our friendly and dynamic team in our Braeside office. Potential career advancement is available if you are ambitious and want to grow into the future with the company.
Key Responsibilities:
Assessment & Planning
Conduct comprehensive assessments using approved tools to identify clients’ strengths, goals, and support needs.
Develop and regularly review individualised care plans aligned with the Support at Home framework and consumer goals.
Collaborate with multidisciplinary teams, families, and advocates to inform planning.
Care Coordination
Monitor service delivery to ensure it aligns with care plans and budget allocations.
Coordinate referrals and liaise with allied health, nursing, social work, and personal care providers.
Support transitions of care, including hospital discharges and entry into residential care if needed.
Consumer Engagement
Act as a key point of contact for clients and families, ensuring they understand their choices and rights.
Promote client independence, inclusion, and dignity.
Ensure cultural safety and sensitivity in all interactions.
Compliance & Quality Assurance
Maintain accurate, timely records in line with privacy and documentation standards.
Ensure care is compliant with Support at Home Program guidelines and the Aged Care Quality Standards.
Participate in audits, reviews, and continuous improvement initiatives.
Financial Accountability
Work within individualised budgets, ensuring cost-effective service delivery.
Assist clients in understanding their budgets and navigating their service entitlements.
Workforce Collaboration
Provide support and guidance to care workers and support staff.
Participate in training, supervision, and team meetings.
Escalate clinical risks or complex needs appropriately.
Required Qualifications & Experience:
Tertiary qualification in Nursing, Social Work, Allied Health, or related discipline.
Minimum 2 years of experience in home care Case management position.
Knowledge of aged care reforms, Support at Home program design, and My Aged Care systems.
Demonstrated experience in goal-based planning, assessment, and service coordination.
Key Performance Indicators (KPIs):
Timely completion of assessments and care plans.
Client satisfaction and achievement of personal goals.
Compliance with documentation and regulatory standards.
Positive engagement with care staff and health professionals.
Budget adherence and service delivery within funding allocations.
