About the role...
HYPAR is a NDIS registered support service providing Mentoring/Disability Support, Supported Independent Living (SIL), Respite/Short Term Accommodation (STA) and Psychosocial Disability Support services through community access and daily living at a pace of interaction guided by the participant.
The NDIS Team Leader oversees daily operations and employees to ensure quality support aligns with participant plans and NDIS standards. They manage rosters, guide Mentors, and liaise with participants and stakeholders to meet individual goals.
The NDIS Team Leader position will be based at our head office in Narangba, but will support the team and participants throughout the Greater Brisbane region.
Key Responsibilities
Case management of Community Access and SIL participants per case load allocation.
Develop case note summaries and monthly support summaries.
Support House Supervisor/s to oversee daily/weekly SIL Participant operations as per their NDIS agreement/Plan, and goals.
Collaborate with the Customer Relations Team, with regard to Participant Intakes: Including ROC’s, Service Agreements and Schedules of Support.
Complete care plans and risk assessments of assigned Participants.
With guidance from the Program Coordinator, conduct performance coaching, development and succession planning and regular employee supervision.
Oversee rostering function, manage shift fills and changes on a daily basis.
Liaise with relevant stakeholders, including but not limited to funding bodies, NDIS, Parents, Caregivers, attending stakeholder meetings as required.
Advocate for the Participant by assisting with referral to psychologists, psychiatrists, counsellors, education bodies, legal representation, youth justice, accommodation, Centrelink, Public Trust and Public Guardian and other specialists as required.
Approve timesheets inline with payroll processing timeframes.
Any other duties as directed by management.
The ideal candidate for this position will have:

A solid understanding of NDIS practice standards and framework
You will also be required to have or be willing to obtain:
Minimum Certificate IV or above in an industry relevant area (youth work, disabilities, community services, counselling etc)
Previous experience in rostering is highly regarded.
Case management experience highly regarded.
Knowledge of NDIS funding, price guide and Service Agreements, Community Access & SIL.
High level organisational and computer skills with ability to juggle competing demands and meet firm deadlines
