Practice Nurse – Chronic Disease Management

The Hub Medical Centre Burpengary is a well-established medical centre located in Burpengary East. We have a dynamic team of 9 doctors and allied health practitioners on site, including, psychologist, physiotherapist, podiatrist, diabetic educator, dietician and visiting consultants.

We are looking for an additional part-time experienced Treatment Room/ Chronic Disease Management (CDM) nurse to join our busy and professional team. This role will see you working within our busy treatment room and within our CDM team to support GPs and their patients in managing all health conditions. You will be working 2-3 days per week, in a small well supported team.

The Role

Conduct health checks and assessments to identify those at high risk of chronic disease or hospital admission.

Prepare care plans and team care arrangements to help patients meet their health and wellbeing goals.

Coach patients on health behaviour change.

Work collaboratively with GPs to promote preventive health care.

Clinical patient care – including acute care, wound care, skin cancer medicine, immunisations, ECG, ABI, and spirometry within our busy treatment room.

Training in our systems and care plan program will be provided to ensure consistency across our service.

Selection Criteria

Our successful candidate will have:

AHPRA registration – Registered Nurses preferred.

Ideally 1-2 years of working within CDM in general practice.

Knowledge of Best Practice - preferred, but not essential.

Excellent communication skills

The ability to establish and maintain collaborative working relationships with both patients and team members – GPs, the clinical team and support staff.

Understanding of the diverse role and nature of primary care nursing, the ability to work autonomously while remaining aware of the scope of practice/limitations, and willingness to seek help where required.

Previous experience is preferred, however not essential as full training will be provided.
