Redeema is a trusted and registered NDIS Provider delivering high quality services to meet the evolving needs of our participants and the community. We are experiencing unprecedented growth and demand for our Support Coordination services as a result of our specialty in Hospital Transitional Supports for high intensity and complex care need participants. We are seeking an experienced Social Worker or Allied Health Professional (minimum of 3 years experience) in the role of a Support Coordinator/Psychosocial Recovery Coach, renumerated with a base salary between $86,000-$91,0000 and an additional 11.5% Superannuation on a full-time, 12 month contract. This position will be responsible for delivering KPIs of 30 billable hours weekly. You are expected to work across a caseload mix of both Level 2 and 3 Support Coordination Participants. To provide security in your tenure, the contract will roll over each year until the NDIS consolidates its position on Support Coordination going forward.
As a seasoned practitioner, you will be required to commence immediately or within a 2 week timeline. We offer a contemporary work environment with a flexible/hybrid model with 2 days in head office at Port Melbourne; this will allow for supervision, training, group work and to attend networking/professional development events. The position is focused on working with participants in our growth areas in the Western and Northern Metropolitan Region with a focus on facilitating hospital transitions for participants with complex and high intensity needs.

About The Role

As the first point of contact for participant you will:
play a vital role in creating positive experiences
your exceptional communication skills, both oral and written, will allow you to effortlessly receive and respond to phone calls, emails and in person communication, ensuring every interaction is professional and helpful.
conduct assessments and develop individualized support plans in collaboration with participants
assist participants in understanding and accessing the NDIS, including preparing and submitting relevant documentation
coordinate and facilitate the delivery of support services in accordance with participants needs and preferences while effectively managing their NDIS funding
liaise with service providers and stakeholders to ensure seamless service delivery
advocate for participants' rights and preferences, ensuring their needs are met and their goals are achieved
monitor and review the progress and effectiveness of support plans, making adjustments as required
provide information, advice, and guidance to participants and their families regarding available support options and resources
maintain accurate and up-to-date records, ensuring compliance with relevant regulations and standards

To be considered for this exciting role, you should have:
Compulsory - you have significant work experience working with Hospital DLOs & NDIS Leads, Allied Health Practitioners and Rehabilitation wards for participants transitioning from Hospital
Compulsory Qualifications- Professional Social Worker, Registered Nurse or Allied Health Practitioner
have an established network of providers
well established planning principles and focus on health practice and methodology for complex and high intensity participants
minimum 3 years experience as a Support Coordinator
have a positive customer focus
dedication to teamwork
ability to work in a professional manner
strong computer literacy skills, including proficiency in Microsoft Office Suite
a thorough understanding of NDIS, statutory and legislative obligations

Ability to initiate and maintain contacts with a broad range of people and community organizations to ensure clients' access to services, appropriate communication with fellow workers, families and other people involved with the service and in the life of the and their carer/s

Excellent communication skills, both written and verbal and the ability to communicate in a positive and collaborative way
Demonstrated ability to manage financial resources in a complex environment
Excellent initiative and self-directed work practices as well as highly developed organizational skills
Desirable Skills
Compulsory- Degree in Social Work , Nursing or Allied Health Professional (ie.OT, Physio). This is not negotiable based on the caseload demand for Level 3 SC requisites
A minimum of 3 years experience as a Support Coordinator or Recovery Coach
Understanding of issues facing the NDIS and disability sector
Experience in working with the complex disabilities and participants with high intensity
A well established reputation and network with various metropolitans hospitals for discharge planning of participants with complex care needs
